PhilHealth is accepting requests from members who want to move to a different Yaman ng Kalusugan Program (YAKAP) clinic until December 31, 2026.
The filing period began October 1. Members and their qualified dependents can use it to reassign their primary care profile to a YAKAP clinic that is closer to where they live or better suited to their medical needs.
PhilHealth Regional Office NCR Vice President Dr. Bernadette C. Lico said the program is built on the 4-Cs of Primary Care: First Contact of Care, Continuous Care, Comprehensive Care, and Coordinated Care.
“Our network ensures you have a dedicated primary care supporting your health needs across every stage of life,” Lico said.
How to file a transfer
Members must file their transfer request in person at any PhilHealth Local Health Insurance Office (LHIO) so their benefits remain active. PhilHealth asks applicants to bring their PhilSys National ID or their PhilSys registration.
If PhilSys verification cannot be completed right away because documentation is still pending, members may present a valid government-issued ID or an acceptable alternative document. They must also submit a duly accomplished Primary Care Selection Form.
Members who cannot apply in person may send an authorized representative. The representative must bring a signed authorization letter from the beneficiary and valid proof of identity for both the member and the representative.
Once a request is processed, the member receives an email confirming the new clinic assignment. PhilHealth said this keeps access to primary care benefits in place, including regular consultations, laboratory tests, prescribed medicines, and coordinated medical referrals.
YAKAP payouts
PhilHealth also released updated figures for the program. As of October 7, 2026, total YAKAP claims payouts nationwide reached ₱7.44 billion, with a 98.37 percent claims payment rate. Private YAKAP clinics received ₱1.92 billion, while government YAKAP facilities received ₱5.52 billion.
As of October 2, 2026, PhilHealth had paid ₱1.57 billion in claims under the Guaranteed and Accessible Medications for Outpatient Treatment (GAMOT) program, which covers up to ₱20,000 worth of essential medicines per year at no cost to members. The agency reported a claims processing turnaround time of nine days.
PhilHealth thanked its accredited YAKAP providers and the members who use the program. The transfer window closes on December 31, 2026.
