When to Update Your PhilHealth MDR
You should update your PhilHealth MDR if there are significant changes in your personal information or if there is incorrect or misspelled data. The following are some reasons why you should update your PhilHealth membership information:

Step 1: Download and Print the PMRF

Step 2: Fill Out the PMRF
After printing the PMRF on A4 paper, it’s time to fill out the form. Using a black ballpoint pen, write all the required information in UPPER CASE/CAPITAL LETTERS. If the information is not applicable, write “N/A.”
- PhilHealth Identification Number – Write your 12-digit PhilHealth number. Read this article if you lost or forgot your PhilHealth number.
- Purpose – Check “Updating/Amendment” only.

I. Personal Details
- Member – Write your last name, first name, name extension (e.g. Sr., Jr.), and middle name. Check the boxes for “No Middle Name” and “Mononym” if applicable (mononym means your name is only one word).
- Mother’s Maiden Name – Write your mother’s complete name before her marriage and as it appears on your birth certificate.
- Spouse – Write your husband’s or wife’s complete name.
- Date of Birth – Write your date of birth in MM/DD/YYYY format. For example, June 30, 1990 is 06/30/1990.
- Place of Birth – Write the city/municipality and province where you were born. Include the country if you were born outside the Philippines.
- Sex – Choose either “Male” or “Female.”
- Civil Status – Select whether you are “Single,” “Married,” “Legally Separated,” “Annulled,” or “Widow/er.”
- Citizenship – Check “Filipino” if you’re a citizen of the Philippines.
- PhilSys ID Number (Optional) – Write your Philippine National ID number if you have one.
- Taxpayer Identification Number (TIN) (Optional) – Write your 9-digit TIN if you have one.
II. Address and Contact Details
- Permanent Home Address – Write your permanent home address including unit/room no./floor, building name, lot/block/phase/house number, street, subdivision, barangay, municipality/city, province/state/country, and ZIP code.
- Mailing Address – Check “Same as Above” if your mailing address is the same as your home address. If not, write down your mailing address.
- Home Phone Number – Provide your home telephone number including country code (63) and area code. For example: (63)(32)411-1234. Write “N/A” if you don’t have one.
- Mobile Number (Required) – Write your 11-digit mobile number.
- Business (Direct Line) – Indicate your business telephone or landline number. Write “N/A” if you don’t have one.
- E-mail Address (Required for OFW) – Your e-mail address should be written in capital letters (e.g. [email protected]).
III. Declaration of Dependents
If you would like to add dependents, include them in the fields provided. Provide the full name of your spouse, children below 21 years of age, and parents who are 60 years old and above and totally dependent on the member.

- First Name
- Name Extension – Example: Jr., Sr., III.
- Middle Name
- Relationship – Indicate your relationship with the dependent (e.g. daughter).
- Date of Birth – Write the dependent’s date of birth in MM/DD/YYYY format. For example, June 30, 1990 is 06/30/1990.
- Citizenship – Write “FILIPINO” if dependent is a Philippine citizen.
You can also check the boxes for “No Middle Name,” “Mononym,” and “Permanent Disability” if applicable.
IV. Member Type
- Direct Contributor – Select your membership type as a direct contributor, meaning you or your employer is directly contributing to PhilHealth. For example, if you are an employee of a private company, check the box for “Employed Private.” If you’re freelancer, check “Self-Earning Individual.”
- Indirect Contributor – Select your membership type if you’re an indirect contributor, which means that your contributions are subsidized by the government. For example, if you’re a beneficiary of the Pantawid Pamilyang Pilipino Program (4Ps), check the box for “4Ps/MCCT.”
- Profession – Specify your profession unless you’re employed, a lifetime member or a sea-based migrant worker.
- Monthly Income – If you are a self-earning individual, kasambahay or family driver, indicate your monthly income in Philippine pesos.
- Proof of Income – Indicate the proof of income that you will submit as a supporting document (e.g. payslip).
In this section, check the box corresponding to the type of updating or amendment in your PhilHealth MDR. Write your old information in the “FROM” field and your new or updated information in the “TO” field.
- Change/Correction of Name – Check this if you want to correct an erroneous entry or misspelling of your name, if you had a change of name due to marriage, or if you have a legal change of name.
- Correction of Date of Birth – Check this if you want to correct an erroneous entry in your date of birth.
- Correction of Sex – Check this if you want to correct an erroneous entry in your sex or gender.
- Change of Civil Status – Check this if you recently had a change of civil status (e.g. single to married).
- Updating of Personal Information/Address/Telephone Number/Mobile Number/E-mail Address – For any changes and updates in your personal information, check this box and enter both the old and new information.
Finally, affix your signature over your printed name and indicate the date when the PMRF was signed. If unable to write, affix your right thumbmark on the space provided.
Step 3: Scan or Take a Photo of Your PMRF
After filling out the PMRF, scan or take a photo of the document and save it as a JPG or PDF file. If you don’t have a scanner, you can use a scanner app like CamScanner (download from Google Play or the App Store).
Make sure that you have scanned or photographed both pages of the PMRF, and that all the information in the form are clear and legible.
If you have supporting documents, such as a birth certificate or marriage contract, scan or take a photo of them as well. Include your valid ID as PhilHealth will need it to verify your identity.
Step 4: Email Your PMRF to the Local PhilHealth Office
Once your PMRF, supporting documents and valid ID are scanned or photographed, it’s time to submit them to your local PhilHealth office via email.
Open your email application (e.g. Gmail) and write the following message (feel free to edit it with your personal information):
I would like to request an update and amendment of my PhilHealth Member Data Record (MDR). Attached to this email are my duly accomplished PhilHealth Member Registration Form (PMRF), supporting documents and valid ID.

Attach the photos or scanned copies of your supporting documents and valid ID, and send the email to your local PhilHealth office with the subject “Updating of PhilHealth MDR.”

Step 4: Wait for PhilHealth to Update Your Records
Now all you have to do is to wait for a reply from PhilHealth acknowledging receipt of your email. Your request will then be forwarded to the processor who will update your records.
Processing time is normally 2-3 working days from the date of acknowledgement. During this time, PhilHealth may ask for additional supporting documents or proof of identity.
You will receive another email from PhilHealth once they have successfully updated your MDR. If there has been no update from PhilHealth after 3 working days, you may call your local PhilHealth office to check the status of your update request.
